Below is a list of insurance companies that we are currently a preferred provider for.  If you have insurance that we are not a provider for, please contact us.  We know that the insurance industry is constantly changing and that employers are always looking for the better deal.  Our regular office fees are with the UCR (Usual, Customary, and Reasonable) rates, which most insurance companies use as their guideline.  We accept insurance payments from all insurance companies that will pay us directly, please contact us to get the specifics.
